Geez - there is no need for him to remove your old fillings! WTF. Leave 'em in until they fail, that's my advice. And the next time the hygienist starts to yank on a filling, get up out of her chair and leave. I fucking hate dentists/doctors/etc who do unnecessary work to line their pockets.
